According to newly released documents from an inspection by the South Carolina Department of Health and Environment Control (DHEC), Phaire’s Care—an Orangeburg nursing home—has a history of issues.
The assisted living facility is currently facing two lawsuits. One is a wrongful death lawsuit after a resident passed away from injuries she suffered from a collision as a passenger of a vehicle driven by Carlton Phare, who is the owner of Phaire’s Care, and another lawsuit accusing the nursing home of medical malpractice after a resident didn’t receive proper medical care from staff members.
Earlier this month, a resident went missing. He was later found the following day, after spending the entire night outside.
The inspection revealed Phaire’s Care doesn’t have a policy which addresses how resident records, evaluations, physical examinations, and room maintenance are managed. Furthermore, there isn’t a policy geared toward medication administration, storage, and security.
DHEC inspectors reviewed staff records and discovered that two out of seven didn’t receive criminal background checks prior to being hired at the nursing home. Inspection documents also state that the facility failed to meet the minimum number of staff members by state standards.
In addition to policy errors, inspectors discovered issues with the facility itself, such as rust stains on vents, restroom stains, broken paper towel and soap dispensers, and a hole in a wall near the entrance door. According to an inspection in January, DHEC documented other issues, like staff members failing to administer medication correctly, failing to maintain patient records, and failing properly feed residents.
